On October 3, 2011, a large fire broke out at the Magnablend Inc. chemical blending and warehousing facility in Waxahachie, Texas, injuring five workers and emergency responders, destroying the facility, and releasing a large toxic plume into the surrounding community. The fire was triggered by an exothermic reaction when incompatible chemicals were inadvertently mixed during blending operations.
Magnablend provided chemical blending and private-label formulation services for industrial and agricultural chemical customers. The company blended a wide variety of chemical products, including materials that were incompatible with each other. The fire began when an incompatible chemical combination was introduced into the blending system.
The CSB found that Magnablend did not have an adequate chemical compatibility management system. Chemical compatibility information for the materials being blended was not consistently available or applied at the point of blending operations.
